ACKNOWLEDGMENT FOR PERSONNEL FILE

I have received and have/will read this Personnel Policy Manual and understand that the City of Kotzebue reserves the right to revise any portion of these policies at its sole discretion, at any time.

I acknowledge that I have received a copy of the City of Kotzebue Personnel Policy Manual. I accept responsibility to read its contents and ask questions if I am uncertain about anything in these policies. I agree to comply with all policies now in existence and developed during employment and understand that failure to comply with any policy may result in disciplinary action up to and including termination.
